Section 477.1 - Initial issuance

(a) All facilities which have been approved by the department prior to September 1, 1974, whether by issuance of a certificate of approval or by issuance of any other instrument which indicates compliance with the regulations of the department, shall be issued an operating certificate without application therefor.

(b) All other facilities shall file a written application for an operating certificate, on a form provided by the department and in accordance with any written instructions of the department pertaining to the procedures for such filing.
