For the purpose of establishing compliance in accordance with this Part, the local department, by using case records, special logs, annotation of inclusions with assistance checks, and other such written statements, shall provide documentation for the following:
(a) the personal offer with date of offer. This shall be so noted in writing by the welfare examiner:
(b) the mailing of the flyer to public-assistance and medically-needy-only families;
(c) the mailing of the form letter to foster parents;
(d) the mailing of the department form letter to public-assistance families at the time of case closing;
(e) the notification in writing to public-assistance applicants at the time the application was denied or withdrawn;
(f) the oral and written requests, with the date each request was made, for social, educational, and medical services, the disposition of each request with the date of implementation; the source from which the services are to be provided; and the method of payment.
