Section 458.5 - Supportive services to residents in other certified facilities

Social services districts, in accordance with section 313.2 of this Title, must provide follow-up visits to and contact persons who are State charges, as defined in section 2 (19) of the Social Services Law and who are residing in residential care facilities. Social services districts must also provide other services, as determined necessary by such districts, to persons in facilities, other than family-type homes for adults which are certified by the department, the Department of Health, the Office of Mental Health or the Office of Mental Retardation and Developmental Disabilities. The specific responsibilities of each social services district include:

(a) monitoring the placement of an individual whose placement in the facility was arranged by the district;

(b) assessing the need for and providing financial management services, such as acting as representative payee to voluntary clients, as determined necessary by the district; and

(c) providing or arranging for other services as determined appropriate by the district.
