Section 456.4 - Outreach for information and referral services

Each local social services department shall make outreach efforts in its community by means of any or all of the following:

(a) public speaking engagements;

(b) stationing of services staff in various community facilities such as schools, hospitals, senior citizen centers, neighborhood centers, and other public and private community agencies;

(c) providing applicants for financial assistance, and recipients of financial assistance at each periodic redetermination of eligibility, with an opportunity to request services and forms prescribed by the State Department of Social Services;

(d) establishing of satellite centers to increase the community's access to services;

(e) in accordance with a plan submitted to and approved by the department, stationing of services staff at Federal social security district offices to provide information and referral services to applicants for supplemental security income and social security benefits.
