Section 421.12 - Application

(a) Authorized agencies operating an adoption program shall use an application form approved by the department to elicit the following information from adoptive applicants:
(1) name, address and other necessary
identifying information;
(2) age,
race and religion of members of the household;
(3) whether applicant is a certified or
licensed foster parent; and
(4)
characteristics of child(ren) sought to be adopted.
(b) Authorized agencies shall develop a record for each adoptive applicant which shall include, but not necessarily be limited to:
(1) record of inquiry maintained
in accordance with section
421.11(i) of this
Part;
(2) the application, medical
report and references;
(3) summary
of interviews with applicant and of visit to applicant's home;
(4) summary of agency conference which
clarifies the basis for each decision that affects the applicant's status with
the agency; and
(5) copies of all
correspondence with the applicant.
(c) Authorized agencies shall register each adoptive applicant who expresses a willingness to adopt a handicapped or hard-to-place child with the adoptive parent registry as required in section 424.2 of this Title.
