(a) Individual recipient basic data file.
(1) A basic data file shall be maintained by
the social services district on each individual services recipient; the file
may be either a manual or an automated one.
(2) The data file shall contain the following
information about the recipient:
(i)
identifying information;
(ii) basis
for eligibility;
(iii) services
provided;
(iv) goal toward which
each service is directed;
(v)
provider agency for each service;
(vi) such other data as the department may
from time to time require; and
(vii) documentation of having met the
criteria for eligibility.
(3) Such data file must be established by
providing the information as prescribed by the department for:
(i) a request or referral for
services;
(ii) authorization for
services; and
(iii) children in
foster care placement, the information as prescribed by the
department.
(4) At time
of recertification the basic data file for each services recipient shall be
updated to reflect changes in circumstances and services provided.
(b) Other records.
(1) A copy of the eligibility notification
letter sent to each service applicant in accordance with section
404.1(f)
of this Title shall be maintained by the social services district.
(2) The social services district shall
maintain for each service recipient such other records as the department may
from time to time direct.
