Part 401 - Comprehensive Annual Social Services Program Plan
Section 401.3 - Submission and approval of the proposed district component

(a) The social services district shall develop and submit annually to the department for approval in a format and at a time specified by the department, a proposed district component appropriate for incorporation by the department as part of the proposed comprehensive annual social services program plan.

(b) The department will review the proposed district component to assure that it is in conformity with minimum Federal and State requirements. The department review will include consideration of programmatic and fiscal feasibility in light of needs and resources identified in the district component. The social services district shall take immediate action in cooperation with the department to correct any deficiencies in the proposed district component identified by the department.

(c) The proposed district component shall describe at a minimum:

(1) the geographic area, indicating at least:
(i) the physical boundaries of the social services district; and

(ii) the demographic, economic, and social characteristics of the social services district.

(2) the organizational structure of the social services district including:
(i) the structure under which social services programs are administered, including a table of organization and functional statement;

(ii) the location of site(s) where individuals may apply for services;

(iii) an indication of the extent of use of volunteers, their functions and how their involvement will be coordinated; and

(iv) the structure, composition and functions of the local advisory council on social services.

(3) needs and resources assessment, including identification of:
(i) data sources used or to be used;

(ii) public and private organizations, including the social services district local advisory council, consulted or to be consulted; and

(iii) the manner in which the results of the needs and resources assessment were utilized in the development of the proposed district component.

(4) program coordination and utilization, including:
(i) coordination with programs under titles IV-A, IV-B, XVI and XIX of the Federal Social Security Act and other related human services programs; and

(ii) steps taken to assure maximum utilization of services provided under the programs described in subparagraph (i) of this paragraph.

(5) planning, evaluation and reporting, including for:
(i) planning--the process of.
(a) coordination with the appropriate budget and legislative processes within the district;

(b) coordination with and input from other local, regional and State planning organizations;

(c) consideration of needs assessment data;

(d) consideration of recources assessment data;

(e) establishment of priorities and program goals;

(f) selection of services to be provided and persons to be served;

(g) fiscal planning and resources management.

(ii) evaluation.
(a) measures of program effort;

(b) measures of program effectiveness and efficiency.

(iii) reporting - such reporting activities as required in section 406.3 of this Title.

(d) the proposed district component shall identify:

(1) Services selected for provision in accordance with section 403.1 of this Title.

(2) categories of individuals to whom each selected service will be made available;

(3) method of provision for each service;

(4) program goals and objectives for each selected service;

(5) estimates of numbers of individuals to be served and funds to be expended within the geographic area specifying:
(i) an estimate of the number of individuals by category to be offered each service;

(ii) an estimate of the planned expenditures for each service by category of individuals to whom the service is to be provided;

(iii) an estimate for all services for the forthcoming program year; and

(iv) a comparison between estimated aggregate non-Federal expenditures for the forthcoming program year and those of the preceding year.

(6) itemization of program resources other than those available under title XX of the Social Security Act to finance social services programs including donated funds.

(e) The social services district shall include such additional information as the department may from time to time require for the development of the district component.
