CAP districts must operate a case management system, directly or through a service provider, which will assist CAP participants to achieve economic and social self-sufficiency. The case management system must follow one or a combination of the following case management processes: single case manager, sequential case management or team case management. The CAP management process must include an ongoing cycle of assessment, planning, linking and monitoring to support the goal of self-sufficiency. Each CAP participant's case management plan must be reviewed, evaluated and renewed every three months.
