The agency is responsible for the completion of all appropriate forms and the securing of medical evidence to be utilized in the determination of an applicant's/recipient's visual status. This material must be forwarded as expeditiously as possible to the Commission for the Blind and Visually Handicapped. The commission must make the determination of visual status. Notification will be returned to the agency.
