(a) Certificate of inspection.

(1) Once every three months, at the time safety valves are set, a certificate of inspection, Form 1-B[FN*] shall be filed in the office of the Department of Transportation, Albany, N.Y., for each locomotive boiler or steam reservoir subject to the jurisdiction of the commissioner. Such certificate of inspection may be a duplicate of the certificate required by the Interstate Commerce Commission on its Form No. 1 entitled "Monthly Locomotive Inspection and Repair Report".

(2) There shall be filed annually with the Department of Transportation a certificate of inspection which may be a duplicate of the certificate required by the Interstate Commerce Commission on its Form No. 3 entitled "Annual Locomotive Inspection and Repair Report". The certificates covering quarterly and annual inspection shall be filed within 10 days after the inspection is made. A copy of each certificate shall be filed with the chief operating officer or employee having charge of the operation of such locomotive boiler or steam reservoir. A copy shall also be placed under glass in a conspicuous place in the cab of the locomotive before the boiler or steam reservoir inspected is put into service.

(3) Each certificate shall give the information required thereon and must be vertified by the oaths of the inspectors.

(b) Specification card. A specification card, Form No. 2-B* containing the results of the calculations made in determining the working pressure and other necessary data shall be filed in the office of the Department of Transportation for each locomotive boiler or steam reservoir, within 10 days after being placed in service in this State. A copy shall also be filed in the office of the chief mechanical officer having charge of the locomotive. Every specification card shall be verified by the oath of the engineer making the calculations and shall be approved by the chief mechanical officer.

(c) Report of alterations.

(1) A report of alterations, Form 222* must be filed whenever alterations are made affecting the strength of a boiler or steam reservoir. This report must also be filed whenever new shell sheets or patches on the shell are applied. Also whenever the number of flues, number or size of safety valves, or the location of water glass or gauge cocks is changed.

(2) Whenever changes are made in the longitudinal seams, a detail drawing should be attached to this report. It is not necessary to file this report when repairs are made to the firebox or flues only.

(3) This report must be filed within 30 days after the alterations are completed.

(4) Whenever a boiler or steam reservoir is removed from a locomotive a report must be filed showing what disposition is made of the boiler.

(d) Reporting washouts. The operator's inspector shall examine the record of washouts on file in the operator's office not less frequently than once every three months, and if he is satisfied of its accuracy he shall enter the date of every washout made during the preceding three months on the certificate of inspection. The inspector must not accept records of washouts unless they conform to the requirements of subdivision (d) of section 923.17.

(e) Withdrawal notice. When a certificate of inspection is due and the boiler or steam reservoir is not inspected, the same must be taken out of service, and a withdrawal notice, Form 104* signed by the official responsible for the operation of the locomotive must be filed within 10 days after the expiration of the certificate of inspection.
