Section 505.3 - Form of notice of violation

A notice of violation shall identify the respondent and be on a form prepared by the commissioner and include, as applicable:

(a) the name and address of the respondent and the case or docket number assigned, if available;

(b) the name and address of the registered owner of the vehicle, if available;

(c) the name and address and driver's license number of the driver of the vehicle, if available;

(d) name and title of the issuer of the notice of violation;

(e) date and location of the issuance of notice of violation;

(f) description of violation;

(g) the amount of penalty or statement that a penalty will be assessed at a hearing; and

(h) such additional matters as may be pertinent.
