The request must include a listing of all successfully trained instructors who are to conduct courses, the location or locations at which the courses are to be given and the proposed fee to be charged, if any, to students who take the course. Such a request should demonstrate that the entity desiring to become a course provider has the facilities and ability to appropriately train students pursuant to the ATV Safety Institute's ATV RiderCourse syllabus.

(c) In order to be a course instructor, a person must be at least 18 years of age, and must have successfully completed the required instructor training program.

(d) The required instructor training program shall be the ATV Safety Institute's Instructor Preparation Course. Information as to the availability of such training programs may be obtained from the ATV Safety Institute, Two Jenner Street, Suite 150, Irvine, CA 92718, Attention: Training Systems.
