An application for fleet registration or renewal shall be made on a permanent fleet registration application form at the Division of Field Services, Permanent Fleet Registration, Swan Street Building, Empire State Plaza, Albany, NY 12228-0432. A registrant does not have to register all of its vehicles as fleet vehicles in order to participate in the fleet registration program, but must register the minimum number established by the commissioner to be eligible.

(a) Documents.

(1) Each registrant of a participating fleet shall be assigned a five digit account number for each address of that fleet or portion of that fleet.

(2) Registrants of vehicles participating in the fleet registration program shall be issued a distinctive vehicle sticker and a distinctive registration document for each vehicle. For passenger and commercial vehicles, the sticker is to be displayed on the inside lower left front windshield. For trailers and special purpose commercial vehicles, the sticker is to be prominently displayed on the outside rear of the vehicle.

(b) Entry. After application is made, the registrant will receive a test tape listing vehicles registered by that registrant. This tape must be reviewed, edited, and returned. The registrant will then be sent a conversion tape of eligible fleet vehicles and a bill. Fee computation will be based on an October 31st date, for proration purposes. There is no proration for the flat $10 fee for special purpose commercial vehicles or for trailers with an unladen weight of less than 1,000 pounds.

(c) Fee payment.

(1) Registration fee payment shall be due by October 31st of each year. A single payment per account must be submitted via tape for all of the fleet's participating vehicles. (See I/O specifications in paragraph [2] of this subdivision.)

(d) Heavy vehicle Federal highway use tax. Registrants are required to submit the appropriate heavy vehicle highway use tax paperwork with the conversion tape and subsequent renewal tapes for all vehicles weighing 55,000 pounds or more.
