Section 26.3 - Procedures for issuance

(a) Where to apply. The original issuance of van pool plates will be restricted to the designated motor vehicle offices. A set of van pool plates will be issued for each vehicle. The plates are embossed "van pool" on the bottom of the plate. All renewal registration transactions for van pool vehicles may be presented at any motor vehicle issuing office.

(b) Required fees and documents.

(1) Application. Each person applying for van pool plates shall submit an application in the form and manner prescribed by the commissioner together with the appropriate fee and certification as set forth in paragraph (4) of this subdivision.

(2) Fee. Vehicles which qualify for van pool plates will be registered at a fee that is the same as the registration fee for passenger vehicles and any motor vehicle use tax prescribed by law will be applied where appropriate.

(3) Insurance certification. Each applicant for van pool plates shall present the appropriate FS insurance identification card.

(4) Certification requirements. Each applicant for an original van pool registration shall certify to the following:
(i) the vehicle is to be used in a van pool operation as defined in section 26.2(a) of this Part;

(ii) the vehicle meets the vehicle size requirements as defined in section 26.2(b) of this Part;

(iii) the vehicle does not operate under any authority of the New York City Taxi and Limousine Commission and/or under any certificate or permit issued by the Department of Transportation pursuant to section 26.2(c) of this Part;

(iv) the route and geographic areas involved in the van pool operation, as well as the name, address and pickup location of the regular van pool passenger location of work place. The employer's name, address and building number must be included, as well as, the names of the regularly travelled highways and arterials;

(v) the approximate one-way trip mileage; and

(vi) the approximate times of first passenger pickup, arrival at the work place, departure from the work place and last passenger discharge.
