Part 594 - Operation Of Licensed Housing Programs For Children And Adolescents With Serious Emotional Disturbances
Section 594.7 - Functional program

(a) A functional program will be developed and submitted with the provider's application for certification. The functional program shall, at a minimum, include:

(1) a description of the agency mission, admission and discharge criteria and the description of the population to be served;

(2) a description of the program's treatment philosophy including values and objectives specifically addressing the issues of resiliency and recovery in a strength-based, family-driven, trauma-sensitive environment;

(3) a description of the specific service needs of the defined target population;

(4) a description of the existing service system, including, but not limited to, mental health, health, social, education, vocational, and other support services;

(5) a description of the residents' utilization or non-utilization of the existing service system and the rationale for such service use patterns;

(6) a description of the gaps in service to the population to be served and the plan to provide or ensure the provision of such services;

(7) a description of the rehabilitative and treatment service methods used to promote successful functioning and integration of residents into family, community, school, employment and independent living;

(8) a description of the goals and anticipated outcomes of the program, including the anticipated average length of stay for residents;

(9) a description of the provider's role, if any, in providing a crisis bed capacity within the geographical area;

(10) a description of the proposed staffing pattern, including job descriptions and qualifications, number of full-time equivalent employees for each position, the supervisory structure and staffing schedule. Such description shall be in the form of a staffing plan;

(11) a description of the services to be offered to families to strengthen positive relationships between youth and families and promote reintegration into the family, as appropriate, including the utilization of family input and involvement in the treatment planning process; and

(12) a description of the steps to be taken to contact and to work with the youth's community school to assist and support an effective educational/vocational program for the youth.

(b) The functional program shall address the use of all necessary services in relationship to achieving the goals of each individual resident.

(c) The functional program shall serve as a basis for delineating anticipated program outcomes which along with program requirements shall be monitored pursuant to section 594.14 of this Part.
