Part 504 - Placement Of Individuals Leaving Transitional Care Funding
Section 504.1 - Background and intent

(a) This Part establishes the process for determining and offering appropriate placement and services in programs licensed and operated by New York State to individuals with mental illness receiving transitional care funding.

(b) This Part establishes standards for an administrative review and informal hearing process should an objection to a placement, which results in a request for an administrative review, be made.

(c) The Office of Mental Health has a responsibility to assist in locating placements and/or services for individuals currently receiving transitional care funding and to complete such placements by January 1, 1999.

(d) The Office of Mental Health has responsibility to assist in locating placements and/or services for an individual reaching 21 years of age while attending a program or residential school which provides services to children, in order that no new transitional care placements occur after July 1, 1996.
