Part 305 - Preventive Counseling Services For Children Of Alcoholics Or Alcohol Abusers
Section 305.5 - Facility requirements

Any facility which provides preventive alcoholism counseling services to a child shall at least:

(a) develop an initial assessment and evaluation to determine the extent of the preventive alcoholism counseling services needed;

(b) prepare a written service plan for the provision of preventive alcoholism counseling services based on individual assessments and evaluations;

(c) provide preventive alcoholism counseling services in accordance with the plan;

(d) maintain a written record of the services provided which shall include periodic notes relating to the child's progress; and

(e) write a closing summary of the preventive alcoholism counseling services provided when it has been determined that such services are no longer needed or when the child is no longer receiving such services.
