Part 100 - State Aid For Operating Costs
Section 100.1 - Applications

(a) Applications from counties, part-counties and cities for State aid for operating costs must be received in quadruplicate in the appropriate regional office of the State Department of Mental Hygiene no later than the day prior to the first day for which State aid will be claimed under the provisions of article 11 of the Mental Hygiene Law.

(b) The application must be accompanied by:

(1) A budget summary which identifies each service and facility and shows the total sum budgeted for each.

(2) Detailed budgets of proposed expenditures as adopted for the local governmental unit.

(3) Copies of contracts for the rendition of services or the operation of facilities by private and/or voluntary agencies.

(4) Copies of agreements or resolutions of the governing body for the rendition of services or operation of facilities by agencies of the city or county other than the local governmental unit.

(5) Copies of the certificates of appointment and the names, terms of office and identification by position or occupation of the members of the community mental health, retardation, and alcoholism services boards.

(c) The budget of the local governmental unit, as submitted, must give a detailed financial statement of each subdivision. This must include titles, annual salary rates or amounts for each position, amounts budgeted for traveling expenses, purchase of equipment, purchase of automobiles or other motor vehicles, rental of space, amounts for other expenses and the expected income from all sources. Such budgets shall be on forms provided by the State Department of Mental Hygiene.
