Section 15.5 - Informal admissions

(a) The commissioner hereby approves all State hospitals and all facilities certified in the hospitals class to receive persons suitable therefor on an informal basis.

(b) Any suitable person over 16 years of age in need of care and treatment requesting admission may be admitted on an informal basis.

(c) A copy of the notice of status and rights given to a person admitted on an informal basis shall be sent to the Mental Health Information Service. The copy of such notice of a person admitted to a general hospital psychiatric unit need not contain the patient's name but, if it does not, it shall include identification of the patient by number and room or ward location to enable the Mental Health Information Service representative to contact him readily.

(d) Form 473 DMH, Acceptance of Informal Admission,is prescribed for use in State and private hospitals. If such form is not used in a general hospital, the patient's record shall clearly show that the patient has been admitted on an informal basis and document the patient's suitability therefor.
