A Certificate Of Approval having been granted by the New York State Drug Abuse Control

Commission to

(Voluntary Agency) to operate a drug abuse treatment and/or pre-

ventive education program, and it having been found that such agency is in violation of the Rules and Regulations of the Commission as follows:

IT IS ORDERED that such Certificate Of Approval is hereby revoked effective ________.

YOU ARE HEREBY GIVEN NOTICE that pursuant to the provisions of Chapter XXV of Title 14 NYCRR 2020.11 you may, within thirty days of the date hereof, request the Commission to grant a hearing to review this revocation of your Certificate of Approval. Any such request shall be made in writing and served upon the Commission at its address shown below.
