For Approval To Operate

Drug Abuse Treatment And/Or

Preventive Education Programs

(Voluntary Agency) having made application to the New York State Drug

Abuse Control Commission for approval to operate a drug abuse treatment and/or preventive education program, and due deliberation having been had thereon, such application is hereby denied for the following reasons:

YOU ARE HEREBY GIVEN NOTICE that pursuant to the provisions of Chapter XXV of Title 14 NYCRR 2020.11 you may, within thirty days of the date hereof, request the Commission to grant a hearing to review this denial of your application. Any such request shall be made in writing and served upon the Commission at its address shown below.
