DETAINER WARRANT

TO ANY PERSON, FACILITY, AGENCY OR JURISDICTION CURRENTLY HAVING A PRESENT RIGHT OF CUSTODY OF ____________

Pursuant to the authority conferred by Article 9 of the Mental Hygiene Law and the regulations of the Narcotic Addiction Control Commission, you are hereby notified that ________, a person now within your custody is subject to a lawful order granting to this Commission custody of his person.

You are therefore commanded, at or shortly before the termination of your period of custody of the said person, to inform the undersigned of the impending termination of such custody and to detain the said person in order that hemay be brought to the actual custody of this Commission, and for so doing this shall be your sufficient Warrant.
