Section 361.4 - Periodic reports

(a) Each self-insurer, and each employer, shall file annually verified reports in form prescribed by the chair, which (without limitations) shall report the following information:

(1) number of eligible employees;

(2) amount of covered payrolls;

(3) amount of employee contributions; and

(4) estimate of amount of employee contributions in the ensuing year.

(b) When the information is deemed necessary, the chair or the Superintendent of Financial Services may require the self-insurer to submit reports to the Department of Financial Services as set forth in Department of Financial Services regulations at 11 NYCRR Part 363.

(c) Other reports requested by the chair pursuant to subdivision (b) of this section shall be filed not later than 15 days after notice of such requirement.
