Section 331.3 - Filing of reports

In order to expedite the processing of claims of disabled workers and to avoid, so far as possible, the appearance of psychologists in contested bill proceedings, the rules with respect to the filing of psychological reports by an attending psychologist are here stated:
(a) All psychological reports filed by an attending psychologist must contain the psychologist's authorization certificate number.
(b) Every psychologist shall file the following reports directly with the chair, and also with the employer or the employer's carrier, if known:
(c) Whenever a report is filed with the chair by an attending psychologist after the time within which it is to be filed, as provided herein and in subdivision (4) of section 13-m of the Workers' Compensation Law, has elapsed, the psychologist shall attach thereto a signed and verified statement giving the true reason or reasons why he or she requests to be excused for late filing.
(d) The following forms are prescribed for the use of psychologists in filing the required reports:
