Section 317.15 - Changes in legal status of group members

Any group member which undergoes any changes in its legal status which results in the issuance of a new taxpayer identification number or unemployment insurance number shall be required to complete a new participation application and shall execute a new participation agreement. A group self-insurer shall, within 10 days, notify the chair in writing of any such change and shall, at that time, file the new application and agreement.
