(a) An organization to whom a certificate of approval has been issued by the commissioner shall maintain records showing the following for each minor to whom the organization had issued a certificate of completion.

(1) Name, address and age of minor.

(2) Date minor entered program and attendance record.

(3) The number of hours the minor participated in the work training program and the number of hours the minor received specific training in safety.

(4) The occupation and work processes for which the certificate of completion was issued.

(b) The organization shall maintain accurate records of injuries and illnesses. Within 48 hours of an accident or injury, other than minor injuries requiring only first aid treatment, the organization shall file a report of the accident with the commissioner.
