Part 201 - Notice Of Changes Affecting Employee Welfare Funds
Section 201.1 - When notice is required

The trustees of every employee welfare fund shall give written notice to the Insurance Department of:
(a) Any of the following changes, within 10 days after it shall have occurred:
(1) any
change in the address of the fund;
(2) any amendment to, substitute for, or
other change in:
(i) the plan;
(ii) the trust indenture;
(iii) any separate trust indenture or other
instrument appointing a banking organization to act as trustee or agent for the
fund;
(iv) any insurance contract
providing benefits to covered employees;
(v) the provision or provisions of any
collective bargaining agreement relating to the fund; and
(vi) any other agreement, regulation, rule,
order or bylaw pursuant to which the fund is operated;
(3) any change in the method of funding
(e.g., from insured to uninsured, etc.); and
(4) any change in the membership of the board
of trustees or other like group having the overall management of a fund, or in
its administrator or other person charged with the administration of the fund
under the direction of such board of trustees or other like group.
(b) Any proposed merger, consolidation, liquidation or dissolution of the fund, within 30 days prior to its proposed effective date.
