Section 17.2 - Applicability of standard claim forms

All commercial insurers providing accident and health insurance in New York State and all article IX-C corporations shall accept the standard claim forms established by the Superintendent of Insurance when submitted by physicians, dentists or by a hospital for services rendered in New York State to an individual covered by an accident and health policy, or when submitted directly by an insured for covered services.
