Appendix 14 to regulation No. 68 has been revised to reflect various changes pursuant to the 19th Amendment to regulation No. 68 (11NYCRR Part 65).
The forms that have been revised are contained herein. They have been reduced in size for convenience of promulgation. The forms must be reprinted in a size and format using type which is clear and readable and which allows sufficient space to facilitate the completion of the information requested herein.
Until December 31, 1985, insurers may continue to use supplies of the superseded forms, provided that all revisions contained herein are affixed by use of a label or an appropriate attachment.
