Section 8.370.19.9 - INITIAL APPLICATIONS

A. All initial applications shall be made on forms provided by the licensing authority.
B. Shall be fully completed.
C. Signed by the person who shall be the licensee.
D. And shall be notarized.
E. All initial applications shall be accompanied by a resume and three character references for the person in charge of the day-to-day operation of the hospice.
(1) References shall not be from a relative
or employee.
(2) License fees are
authorized by law, and will be payable to the extent, if any, set out by other
licensing authority regulations.
