Section 8.100.130.12 - QUESTIONABLE INFORMATION/VERIFICATION

A. To be considered questionable, incomplete or inadequate, the information or verification must be documented as one of the following:
(1) inconsistent with statements
made by the applicant/recipient;
(2) inconsistent with other information on
the application or previous applications;
(3) inconsistent with credible information
received by ISD;
(4) questionable
on its face.
B. Resolving questionable information: Upon receiving questionable, incomplete or inadequate verification needed to determine an applicant/recipient's eligibility or benefit amount, ISD shall promptly provide the applicant/ recipient a notice which shall include the following:
(1) advise
the applicant/recipient of the receipt of the information;
(2) why it is questionable, incomplete or
inadequate;
(3) the additional
information that must be provided;
(4) the alternative methods of providing the
information,
(5) the deadline for
supplying the information (10 working days or the end of the applicable
application processing time period, whichever is later);
(6) that the applicant/recipient may discuss
with ISD whether any other readily available verification is
acceptable;
(7) that ISD is
available to assist the applicant/ recipient if the information is not readily
available; and
(8) that a failure
to supply the needed information or contact ISD by the deadline may result in a
delay, a denial of eligibility, a reduction in the amount of benefits or
termination of benefits.
