Section 5:74-2.5 - Replacement of certificate

The Division shall issue, upon application, a duplicate certificate of any type and specialty upon a finding that the certification has been issued and the applicant is entitled to such certification to replace the one which has been lost, destroyed, or mutilated. Payment of a fee as specified in 5:74-2.10 shall be required.
