(a) The facility shall notify the Department immediately by telephone at (609) 633-6251 followed within 72 hours by written confirmation of the following:
1. Termination of employment of the
administrator, and the name and qualifications of his or her
replacement;
2. All residents who
are missing for 24 hours; and
3.
All suspected cases of resident abuse or exploitation that have been reported
to the State of New Jersey Office of the Ombudsman for the Institutionalized
Elderly and/or to the county welfare agencies.
