The owner shall designate an office or location as a site for reporting accidents or injuries. The office shall be open during normal business hours and shall be marked with clear signage. Forms with the minimum required information (see 5:14A-4.13(c) ) shall be available for riders to fill out at this location. More than one office or location may be required so that the locations are within reasonable walking distance from any ride.
