(a) The facility shall maintain on file the following:
1. A copy of the registration of each
vehicle used to transport children;
2. A
copy of the insurance policy for every vehicle owned, leased, contracted or utilized
by the facility; and
3. The name and
address of the lessor or contractor furnishing a vehicle to the facility, if
applicable.
(b) The facility shall maintain maintenance records for all vehicles used by the facility for the transportation of children, including repair and inspection records, and shall retain them for the lifetime of the vehicles.
(c) The facility shall develop and maintain on file a record of all trips other than routine school-related, work-related, or recreation trips where the facility's vehicles are used for transporting children that documents:
1. The date and time of the trip
and staff member(s) who drove;
2. Names
of the children who attended the trip; and
3. Any unusual incidents that occurred on the
trip.
