Section 2:3-1.4 - Contents of Certificate of Veterinary Inspection

(a) The official Certificate of Veterinary Inspection shall indicate that the livestock or poultry designated thereon comply with all requirements for entry into New Jersey.

(b) The official Certificate of Veterinary Inspection shall include a legible report of the following:

1. Complete name and address of consignor;

2. Origin of the livestock;

3. Complete name and address of consignee;

4. Destination of the livestock;

5. Description of the livestock which shall include: species, breed, sex, and age of animals. Where applicable, the description shall include the following:
i. Cattle and swine shall be identified by ear tag, tattoo or registration name and number or other identification as approved by the State Veterinarian.

ii. Horses shall be identified by physical description, tattoo or brand, or microchip, or other identification as approved by the State Veterinarian.

iii. Sheep and goats shall be identified using the identification method for sheep and goats at Title 9, Code of Federal Regulations, Chapter 1, Subchapter C, Part 79--Scrapie in Sheep and Goats, which is hereby adopted herein by reference.

6. A statement that the examining veterinarian personally inspected the animals described and found them free from visible symptoms of infectious, contagious and/or communicable disease or known exposure thereto within 30 days of shipment;

7. Results from all required tests, or a statement of exemption from required testing and reason for exemption, for each specific class of livestock shall be recorded on the Certificate of Veterinary Inspection; and

8. Additional information as required for each specific class of livestock or poultry.
