(a) The entity shall demonstrate that it continues to meet the requirements for licensure pursuant to N.J.S.A. 5:12-92.a and b and, in furtherance thereof, shall submit, every five years after initial licensure, such information and documentation as the Division may require, including, but not limited to, the information required pursuant to N.J.A.C. 13:69A-5.11A.

(b) Following notification to a licensee or qualifier of such license by the Division of the obligation to file for resubmission and the expiration of the license five years after the license was issued, the Division may administratively revoke this license or qualification of the licensee or any qualifier of such licensee that fails to timely file such information and documentation or notifies the Division of its intention to not file.

(c) An entity or individual whose license or qualification is administratively revoked pursuant to

(b) above shall not be permitted to reapply for any license or qualification status issued by the Division for a period of one year from the date of administrative revocation, except that the Director may, for good cause shown, permit reapplication at an earlier date.
