(a) The following information and data shall be provided for each excluded person:
1. The full name and all aliases the person
is believed to have used;
2. A
description of the person's physical appearance, including height, weight, type
of build, color of hair and eyes and any other physical characteristics which
may assist in the identification of the person;
3. The date of birth;
4. The effective date of the order mandating
the exclusion or ejectment of the person named therein;
5. A photograph, if obtainable, and the date
thereof; and
6. Such other
miscellaneous information as is deemed necessary by the Division for the
enforcement of this subchapter.
