Section 13:69A-15.3 - Contents of an application to convert a casino employee license to a casino employee registration

(a) An application to convert a casino employee license to a casino employee registration shall include:
1. A completed
Casino Employee License Conversion Application, as set forth in
13:69A-5.5C, signed by the
applicant and notarized by a person authorized to administer oaths. This form
shall contain all information since the applicant's initial application for
licensure or action date on that license's last submission;
2. A Release Authorization signed by the
applicant and notarized by a person authorized to administer oaths in the State
of New Jersey, which Release Authorization shall direct all courts, probation
departments, selective service boards, employers, education institutions,
banks, financial and other institutions, and all governmental agencies,
Federal, state and local, both domestic and foreign, to release any and all
information pertaining to the applicant as requested by the Division;
3. Payment of the appropriate registration
fee due in accordance with
13:69A-9.16; and
4. The documents required for identification
by 13:69A-7.2A.
