(a) The Division shall charge the following non-refundable temporary help service certification fees:
| 1. Annual certification, primary location ............................... | $ 250.00; |
| 2. Annual certification, secondary location ............................ | $ 250.00; |
| 3. Late fee ....................................................................... | $ 25.00 (for each 30-day period or portion thereof that the application is late). |
