(a) An applicant for licensure as an LSW shall submit the following, on forms provided by the Board:
1. A completed application form, which
requests information concerning the applicant's educational and experiential
background;
2. The application fee
set forth in
13:44G-14.1;
3. A signed consent form acknowledging that
the Board will perform a criminal history background check as a pre-requisite
of licensure;
4. An official
transcript or letter from the dean or registrar of the school indicating that
the applicant has received a master's degree in social work from an educational
program accredited, or in candidacy for accreditation, by the Council on Social
Work Education or a doctorate in social work from an accredited institution of
higher education; and
5. Proof of
successful completion of the master's level examination administered by the
ASWB.
i. An applicant pursuing a master's
degree in social work and enrolled in the last semester of an educational
program accredited or in candidacy for accreditation by the Council on Social
Work Education is eligible to take the master's level examination.
