Section 13:44C-7.2B - Reporting to Department of Health and Senior Services

(a) Pursuant to 8:19-1.10, a licensee who conducts outpatient screening or audiologic testing required by 8:19-1.9 on a child shall complete the New Jersey Department of Health and Senior Services' Newborn Hearing Follow-up Report form, submit the report to the Department of Health and Senior Services and report the results of the testing to the child's primary care provider within 10 days of the test being performed.

(b) Pursuant to 8:19-1.11, when a licensee confirms a permanent hearing loss, he or she shall complete and submit to the New Jersey Department of Health and Senior Services:

1. A Newborn Hearing Follow-up Report form (form number SCH-2), for children up to 36 months old; and

2. A Special Child Health Services Registration form (form number SCH-0), for children up through 21 years of age.
