(a) Applications for licensure may be obtained at the office of the Advisory Committee, 124 Halsey Street, Newark, New Jersey 07101. The Committee's mailing address is Audiology and Speech-Language Pathology Advisory Committee, Post Office Box 45002, Newark, New Jersey 07101. The Committee's website is www.nj.gov/oag/ca/medical/audiology.htm.

(b) All applications for licensure shall be accompanied by the fee as set forth in 13:44C-2.2.
