The following numerical codes will designate the indicated color of eyes and the licensee's weight:
| EYE COLOR | WEIGHT |
| 1--Black | 0--Under 121 pounds |
| 2--Brown | 1--121 to 140 pounds |
| 3--Gray | 2--141 to 160 pounds |
| 4--Blue | 3--161 to 180 pounds |
| 5--Hazel | 4--181 to 200 pounds |
| 6--Green | 5--201 to 220 pounds |
| 7--Special code | 6--Over 220 pounds |
| 8--Special code | |
| 9--Special code |
