Section 11:21-7.12 - Reporting requirements

(a) A small employer carrier shall file with the Board, quarterly no later than 60 days after the end of the fiscal quarter, the following information reported with respect to standard health benefits plans:
1.
The number of small employers, covered employees, and dependents that were
issued health benefits plans in the previous calendar quarter, reported
separately as to newly issued plans and renewals, and separately for standard
health benefits plans B, C, D, and E; plans B, C, D, and E sold through or in
conjunction with a selective contracting arrangement; HMO; and HMO POS;
and
2. The total number of health
benefits plans in force at the end of the quarter, and the total number of
employees and dependents covered, reported separately for each standard health
benefits plan B, C, D, and E; plans B, C, D, and E sold through or in
conjunction with a selective contracting arrangement; HMO; and HMO
POS.
(b) Quarterly reports shall be filed at the address listed in 11:21-1.3.
(c) An insurance company, health service corporation, hospital service corporation, or medical service corporation and affiliated health maintenance organization shall file separate reports.
