The parent, guardian, caretaker or authorized agent of a presumptively eligible child shall be contacted by the county welfare agency or Statewide eligibility determination agency, as appropriate, during the presumptive eligibility period to provide any additional information required to complete the application process. As part of the eligibility determination process for NJ FamilyCare-Children's Program, the parent, guardian, caretaker or authorized agent of a presumptively eligible child shall complete any forms required as a part of the application process, and shall assist the eligibility determination agency in securing evidence that verifies eligibility.
