(a) A psychiatric facility shall report to the Department, by a written report to the Assistant Commissioner for Mental Health and Addiction Services, every incident in which an employee has materially failed to comply with the policies required by 10:32-1.5.
1. Notification of the Assistant Commissioner
shall occur no later than five business days after the facility substantiates
the event and shall be made in a form and manner prescribed by the Division.
