(a) Pharmacies shall verify that the beneficiary is a Senior Gold covered person. This is done by checking the beneficiary's Senior Gold identification card or by calling the Recipient Eligibility Verification System (REVS) at 1-800-676-6562.
(b) The Program shall issue to all Senior Gold eligible beneficiaries a Validation Identification Card. The document shall contain the patient's name, Senior Gold identification number, effective date and expiration date.
(c) The beneficiary is eligible only for the period of time indicated on the identification card.
