(a) Whenever the board determines that a disciplinary hearing shall be held, the parties shall be given at least 15 days' written notice of such hearing. The notice shall be served on the parties either personally or by certified mail, return receipt requested.
(b) The notice shall include:
(1) The time, place, and nature of the
hearing;
(2) A statement of the
legal authority under which the hearing is held;
(3) A reference to the particular sections of
the statutes and rules involved;
(4) A short and plain statement of the issues
involved;
(5) A statement that each
party has the right to have an attorney present to represent the party at the
party's expense; and
(6) A
statement that each party has the right to have the board provide a certified
shorthand court reporter at the party's expense and that any such request be
submitted in writing at least 10 days prior to the proceeding.
