(a) The department shall notify the individual license holder by first class mail, at least 30 days prior to the expiration of a license, of the following:
(1) The date that the license expires;
and
(2) The requirements for the
renewal of the license, pursuant to these rules.
(b) All renewal notices shall be sent to the license holder's last known address.
